Registration Requirements

Registration Process:

Our registration process is simple, secure, and entirely online. To complete your child's enrollment, please ensure the following documents are scanned and emailed to info@maplerootacademy.com. No physical forms will be accepted in person. Incomplete submissions may delay the registration process.

Required Documents:

1. Previous School Records:

These must be sent directly to Maple Root Academy or provided in a sealed envelope.

• Grades and Behaviour Records
• Health Records
• Any other relevant documents

2. Personal Documents:

• Birth Certificate
• Updated Health Records
• Custodial Documents (if applicable)

3. Maple Root Academy Forms and Fees:

• Completed Maple Root Academy Registration Package
• One-time fees: $150 registration fee, $200 service fee, and $60 yearly uniform fee (per child)
• Tuition Balance
